Welcome to the Bent Tree Bluffs Stingrays 2023 Registration Form!

We look forward to this summer as we get back into the pool!

As a reminder for returning swimmers and a heads up for new swimmers - at least one parent or guardian will be expected to volunteer at each swim meet. The meets cannot be completed without the adults volunteering at each meet and your participation is highly appreciated.

Participation Waiver

Our agreement to these waivers and consents is required for registration. Furthermore, any violation of the codes of conduct will be considered a basis for being released from the team with a complete forfeiture of all fees.

Swimmer Release Waiver and Consent

Participation in the Bent Tree Bluffs Stingrays 2023 swim season is completely voluntary. By signing your child up to participate, you agree to the following liability waiver and release of claims on behalf of yourself and your participating child(ren):

I understand that registration for and participation in the Bent Tree Bluffs Stingrays 2023 swim season is completely voluntary. Signing my child up for team membership and allowing him or her to use the Bent Tree Bluffs community pool and its associated amenities is solely my choice. I recognize and voluntarily accept the risks associated with allowing my child to participate in the Bent Tree Bluffs Stingrays 2023 swim season and use the Bent Tree Bluffs community pool, including but not limited to, exposure to the COVID-19 virus, bacteria and other pathogens, swimming injury, and slip or trip and fall.

I agree, on behalf of myself and my child, to waive any claim against and to release Bent Tree Bluffs Stingrays, Suburban Swim League and all of its agents, officers, directors, servants, representatives, volunteers, and employees from any liability whatsoever in connection with my child’s participation in the Bent Tree Bluffs Stingrays 2023 swim season and his or her use of the Bent Tree Bluffs community pool and its associated amenities. The scope of this waiver and release includes, but is not limited to, exposure to COVID-19 virus, bacteria and other pathogens, swimming injury, slip or trip and fall, and any other damages, losses, or injuries encountered in connection with and/or sustained as a result of my child’s participation in the Bent Tree Bluffs Stingrays 2023 swim season. I further agree, on behalf of myself and my child(ren), to indemnify, hold harmless, and release Bent Tree Bluffs Stingrays, Suburban Swim League and all of its agents, officers, directors, servants, representatives, volunteers, and employees from and against any and all claims and causes of action of every kind arising from physical or emotional injuries and/or damages which I or my child(ren) may incur as a result of or relating to his or her participation in the Bent Tree Bluffs Stingrays 2023 swim season and/or use the Bent Tree Bluffs community pool and the amenities associated therewith, including injuries and/or damages sustained because of the negligence of Bent Tree Bluffs Stingrays.

I give my consent for all emergency medical care undertaken by a coach or volunteer, or prescribed by a licensed physician or other health care provider for my swimmer (identified above on registration form). Care may be given under whatever conditions are necessary to preserve the life, limb, or well being of the swimmer.

Registering your child for the Bent Tree Bluffs Stingrays swim team constitutes your acceptance of the above waiver, release, and indemnity agreement and your agreement to be bound by it.

Code of Conduct

Any Member, Parent, Player, Coach, or Spectator participating or attending any meet, practice, or other event affiliated with the BTB Swim Team shall abide by the following Code of Conduct.

Suburban Swim League Parent’s Code of Conduct The Suburban Swim League (“SSL”) seeks to instill positive character building traits in our communities’ youth through the demonstration of good sportsmanship, respect for others, responsibility, fairness, caring and good citizenship. We ask that parents and their guests attending SSL sponsored events help us by reflecting these character traits at swim meets.

When attending SSL sponsored events, I therefore agree:

● I will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy for all athletes, coaches, stroke and turn judges(“officials”), and spectators at every meet and/or practice.

● I will not engage in any kind of unsportsmanlike conduct with any official, coach, athlete, or parent such as using profane language or gestures, etc.

● I will not engage in bullying of any kind and will not tolerate it from my child.

● I will not encourage any behaviors or practices that would endanger the health and wellbeing of the athletes.

● I will teach my child to play by the spirit of the rules and to resolve conflicts without resorting to hostility or violence.

● I will demand that my child treat other athletes, coaches, officials and spectators with respect regardless of race, sex, or ability.

● I will never ridicule or yell at my child or other participant for making a mistake.

● I will respect the officials and their authority during meets and will never question, discuss, or confront coaches or officials at the pool, and will take time to speak with coaches at an agreed upon time and place.

● I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team.

● I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.

● I am aware that alcohol is not permitted at SSL sponsored events due to the leagues insurance policy.

I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following:

● Verbal warning by a team representative, head coach, and/or member of the league organization

● Written warning

● Parental meet suspension with written documentation of incident

● Removal of swimmer from the meet with an official DQ

● Parental season suspension
